We’re at that time of year that we’re going to get a lot of requests for children’s immunization records as parent prepare to register their little ones for kindergarten in the fall.
The easiest and fastest way to get these records (as well as to request medication refills, communicate with your child’s care team, and review notes and results) is to have an active Patient Portal account with us. If you do, follow these instructions to download and print the vaccine records:
- Open the menu on the left side (click on the 3 gray lines in the top left corner)
- Click the + next to My Chart
- Click on Immunizations
- Click the Print button on the right side to download a copy
If you do not have an active portal account for your child, call and talk to our operators who will get you set up quickly and easily.
Of course, you also may request the immunization record by other means, such as having it mailed, faxed, or picking it up in person. PLEASE call us first, however; please don’t show up at the office looking for records as we do not always have the staff present who can help you.
